CAP case managers work with clients to help them achieve healthy and productive lives.  Case managers provide referrals to clients for healthcare and also support them in attending healthcare appointments and adhering to the treatment regimens.  Case managers also provide referrals and assistance around issues such as housing, financial assistance, mental health, substance abuse, public benefits, food assistance, insurance, employment and education. 

 

CAP staff includes bilingual Case Managers who offer assistance in Spanish.

The first step in working with a Case Manager is to become a CAP Client.
